It's a RM-204, used with a KV-1520R. The transducer part number is 8-819-100-00, known as a SUC-1 "Speaker"
I have the service lit - 5 frequencies - 41.5Khz, 38.5Khz, 37Khz, 35.5Khz, and 40Khz. On-off is two oscillators working sequentially, 41.5 first, then the 38.5. Cheers,
